Recovery Care Coordinator - Registered Nurse (RN)

Job Details

  • Employment Type: Permanent
  • Shift: 10-hour shifts, 11 AM - 9 PM with a weekend rotation every 5th to 6th weekend
  • Compensation: $50/HR - $57/HR + $3 shift differential after 7 PM

Job Requirements

  • Associate of Science in Nursing degree or graduate of an accredited Licensed Practical Nursing Program or School of Vocational Nursing
  • Current, unencumbered license to practice as a Registered Nurse (RN), Licensed Vocational Nurse (LVN), or Licensed Practical Nurse (LPN) specific to the state of employment
  • Minimum of 1 year of experience working as an RN, LPN, or LVN
  • Ability to work both day and evening shifts, rotating holidays, and on-call
  • Preferred: 2+ years of experience as an RN, LPN, or LVN, with experience in the Emergency Department (ED), Intensive Care Unit (ICU), or Med Surg

Responsibilities

  • Work directly with patients and their providers for 30 days to deliver acute care at home
  • Identify, present, and discuss potential patients for program inclusion with providers
  • Facilitate communication and coordination among care team members to coordinate admissions
  • Coordinate referrals and resources for continued care in the outpatient setting
  • Maintain required documentation for all patient interactions
  • Coordinate services ordered by partner physicians with the multidisciplinary team
  • Communicate any barriers to care plan fulfillment to appropriate providers
  • Ensure access to services needed for provider-directed care plans
  • Monitor patient care and report to providers as necessary
  • Perform other duties as assigned
