RN Case Manager

Good afternoon. Transitions would like our help with their RN Case Manager opening in the Gurnee/Libertyville territory. Please let me know if you have any questions. Thank you for your help.

RN Case Manager- Hospice- Gurnee/Libertyville area

Need an RN to manage a caseload of approximately 12 – 15 patients per week. Salary is 70-75k but have some flexibility. RN does NOT need experience. Looking for someone compassionate. New grads are welcome. They are a decent size company, census is about 375 right now. Work 12 hours shifts 8A-8P (Attached is a copy of their work schedule. X represents days they are working). They’ll work 3 days one week then 4 days the next. They have a partner that they share their caseload with. Back-up On-call (they have night nurses) a few days per month but never on their days off. $100 per month auto allowance plus a gas card. Company cell and laptop provided. WellSky Software.
