A hospice organization company in the Greater Chicago Areas is seeking Hospice Case Managers (RN), On-Call Hospice RN, and Director of Nursing to join their team due to growth in the Deerfield/Libertyville/Buffalo Grove area.
This organization has a great reputation for the highest quality of service and care to patients. They are accredited by the Accreditation Commission for Health Care (ACHC), which less than 15% of all U.S. hospice agencies meet.
Hospice Case Manager RN:
- You’ll be responsible for a caseload of 12-16 patients per week
- As the Hospice Case Manager, you’ll be responsible for the delivery of the care plan for each hospice patient
- Continuously evaluates personal and professional performance and makes necessary changes to increase productivity and quality of care delivered
- Adheres to and supports the policies/procedures/goals/objectives of hospice to assure quality patient care
- Strives for independence and sound critical decision-making regarding the delivery of patient care in a cost-effective manner without compromising quality
- Develops the patient’s plan of care in collaboration with other professionals and implements treatment strategies based on scientific nursing theory that promotes physical, psychosocial, emotional, and spiritual well-being

Compensation: $85,000 - $90,000
On-Call Hospice RN Position:
- Schedule: On-call every other week (7 days on/7 days off), 5 PM to 9 AM.
- Experience: Hospice or Home Health experience strongly preferred.
- Caseload: Schedule and caseload vary week to week; typical calls involve patient passing, new admissions, or falls.
Requirements:
- RN Licensure
- Previous Home Health, Palliative, Hospice experience preferred
- 2+ years of Acute Care experience minimally required
Director of Nursing Position:
- Maintain effective relationships with management staff, partners, and the general public.
- Participate in establishing and implementing policies and procedures for nursing and personal care services based on accepted standards of practice.
- Coordinate the orientation of personnel to nursing policies and procedures.
- Follow all company policies and procedures, state and federal laws, and regulations, report violations to the appropriate authorities.
- Perform all duties in accordance with the company’s mission, vision, and philosophy.
- Maintain confidentiality of company information.
- Demonstrate honesty and integrity at all times.
- Mileage reimbursement (standard mileage rates).
Job Type: Full-time
Pay: $105,000 - $115,000 per year
