Job Title: Home Health Physical Therapist – Full-Time
Location: Pocono Area
We are seeking motivated and compassionate Physical Therapists to join our Home Health team in the Pocono area. This is a vital role within a close-knit, supportive team, and an opportunity to make a meaningful impact on patients' quality of life. A $25,000 sign-on bonus and relocation assistance are available.

Location: Pocono Area
We are seeking motivated and compassionate Physical Therapists to join our Home Health team in the Pocono area. This is a vital role within a close-knit, supportive team, and an opportunity to make a meaningful impact on patients' quality of life. A $25,000 sign-on bonus and relocation assistance are available.
Schedule & Work Environment:
- Full-time, Monday – Friday
- No weekends
- No holiday shifts required unless volunteering
- Enjoy a comprehensive orientation and training program, complete with mileage reimbursement for all travel during your training period.
Compensation & Benefits:
- Salary based on experience
- $25,000 sign-on bonus
- Mileage reimbursement at 67 cents per mile for patient-related travel
- Competitive bonus incentives for additional visits
Key Responsibilities:
- Provide home health physical therapy services within designated areas, helping patients restore function and achieve independence
- Develop and update individualized treatment plans in collaboration with patients and their families
- Work closely with Rehab Managers and clinical support staff to ensure high-quality care
- Engage with team members in regular communication to coordinate care and scheduling
Qualifications:
- Licensed Physical Therapist with at least 2 years of PT experience; experience in home health or adult/geriatric care preferred
- Proficiency in using electronic health records (experience with EPIC is a plus)
- Strong communication and organizational skills
- Ability to work collaboratively within a structured yet flexible home health setting
