Home Health Physical Therapist - Part Time Weekends

Location: Lehigh and Northampton counties
Schedule: Friday–Sunday or Saturday–Monday, 8:00 AM – 8:30 PM
Commitment: 48 out of 52 weekends per year
Sign-On Bonus: Up to $10,000
Mileage Reimbursement: $0.70/mile (excluding commute from home to first patient and last patient to home)

A leading non-profit healthcare organization is seeking a compassionate and motivated Home Health Physical Therapist to join their well-established weekend team serving adult and geriatric patients. This role offers a stable, supportive environment with excellent tenure and leadership who prioritize mentorship and clinical quality.

What You’ll Do:

• Evaluate, plan, and deliver physical therapy interventions for home health patients

• Develop and regularly update individualized treatment plans in collaboration with patients and families

• Use prescribed exercise, manual therapy, and education to promote functional independence and quality of life

• Maintain timely and accurate documentation using an EMR (Epic experience preferred but not required)

• Collaborate closely with your rehab manager, clinical director, and scheduling staff to coordinate care

• Meet a base productivity expectation of 29 units/week (examples: evaluations 1.5 units, start of care 2.5 units, revisits 1 unit), with competitive incentives for exceeding this

• Participate in up to 10 weeks of orientation depending on prior home health experience, including a combination of in-office, field, and remote training

Qualifications:

• Bachelor’s degree in Physical Therapy (Doctorate preferred)

• Active Pennsylvania Physical Therapist License

• CPR/BCLS certification upon hire

• Valid driver’s license and reliable transportation

• At least one year of physical therapy experience (inpatient, outpatient, or SNF acceptable)

• Adult and geriatric experience required

• Prior home health experience strongly preferred, but not required

• Excellent communication and interpersonal skills to work effectively with patients, caregivers, and interdisciplinary colleagues
