Physical Therapist – Home Health

A highly respected, non-profit healthcare system is seeking a full-time Home Health Physical Therapist to join their team in the Muskegon County MI territory.

As a full-time Home Health Physical Therapist, you will be responsible for providing direct, 1 on 1 care for patients in the comfort of their own home. From heart disease, COPD, and chronic pain to neurological conditions, wound care, fractures and total joint replacements, you will be working with a mixed patient caseload consisting of both adult and geriatric patients with a variety of simple to medically complex conditions and diagnoses.

Caseload: 4-6 patients per day in the Muskegon County territory

Schedule: Full-time, Monday-Friday – 40 hours/wk

Compensation: $65,000 - $95,000/yr (Depending on experience)

Minimum qualifications:

• Possession of a degree, which includes an internship from a baccalaureate/university program in physical therapy approved by the American Physical Therapy Association
• Licensed to practice as a physical therapist in Michigan
• A minimum of one (1) year experience as a physical therapist; home care experience preferred, therapeutic and rehabilitative procedures and techniques, evaluate home environment and make recommendations, and participate in quality improvement processes. Current working knowledge of conditions of participation and home healthcare experience preferred.
• Must have current driver’s license and reliable transportation to and from work site.
