Nurse Practitioner

Job Details

• Employment Type: Permanent, Full-Time
• Hours Per Week: 40
• Shift Duration: 8 Hour Days
• Salary: $120,000 - $150,000 annually

Job Requirements

• Valid unencumbered Nurse Practitioner License for the state of practice
• Previous in-home risk assessment experience preferred
• 3 years of patient care experience preferred (primary care/adult/geriatric, EMR)
• Access to reliable transportation
• Strong ability to work within an EMR system
• Ability to work independently
• Bilingual is a plus

Responsibilities

• Perform annual in-home wellness visits and risk adjustment health assessments
• Provide individualized quality care to the elderly population in their homes
• Deliver patient health education on topics such as pain management and medication
• Build relationships of trust with members through exceptional communication and empathy
• Assist in closing quality care gaps (i.e., screenings and labs)
• Collaborate to enhance care delivery
• Travel throughout the assigned state as required
