About the Role
As an APP on our team, you’ll serve as the clinical lead for your patients—delivering in-home primary, urgent, and palliative care. You’ll work closely with physicians, social workers, paramedics, and care coordinators to ensure each patient receives thoughtful, personalized care. This is a full-time role with a schedule of Monday through Friday, no weekend work required.

Key Responsibilities
- Deliver primary and palliative care to elderly patients in their homes (3–6 patients per day).
- Perform physical assessments, develop care plans, and manage chronic and acute conditions.
- Provide education and counseling to patients and families.
- Manage medications, labs, imaging orders, and referrals as needed.
- Participate in interdisciplinary team meetings and care plan reviews.
- Maintain meticulous documentation to meet care quality and compliance standards.
Experience
- 5+ years of progressive NP or APP clinical experience
- Strong background in geriatrics, internal medicine, palliative care, or end-of-life care
- Home-based care experience preferred but not required
