We are seeking a dedicated Nurse Practitioner for home health visits in New Jersey. This permanent role offers the opportunity to provide personalized healthcare to patients in the comfort of their homes. Job Details

• Salary: $108,000 - $135,000 annually
• Employment Type: Permanent
• Schedule: Monday to Friday, 8:30 AM - 4:30 PM with some flexibility
• Shift Duration: 8 Hour Days
• Territory: Mercer, Atlantic, Gloucester, Burlington, and Camden Counties

Job Requirements

• 1+ years of experience required in healthcare jobs
• Experience in Palliative, Hospice, Geriatric, or Long-Term Care preferred
• Valid Nurse Practitioner license in New Jersey
• Proficiency with Netsmart software is a plus

Responsibilities

• Conduct home visits to see approximately 8 patients per day
• Provide clinical setting visits for up to 10 patients per day
• Participate in on-call duties every 6-7 weeks, primarily over the phone
• Complete documentation and patient care plans using Netsmart software
• Collaborate with a multidisciplinary team to ensure comprehensive patient care
