Nurse Practitioner or Physician Assistant (APP) – Home-Based Primary & Palliative Care

About the Role
As an APP on our team, you’ll serve as the clinical lead for your patients—delivering in-home primary, urgent, and palliative care. You’ll work closely with physicians, social workers, paramedics, and care coordinators to ensure each patient receives thoughtful, personalized care. This is a full-time role with a schedule of Monday through Friday, no weekend work required.
Key Responsibilities
- Deliver primary and palliative care to elderly patients in their homes (3–6 patients per day).
- Perform physical assessments, develop care plans, and manage chronic and acute conditions.
- Provide education and counseling to patients and families.
- Manage medications, labs, imaging orders, and referrals as needed.
- Participate in interdisciplinary team meetings and care plan reviews.
- Maintain meticulous documentation to meet care quality and compliance standards
Job Requirements
- Minimum Master’s Degree in Nursing
- Valid Nurse Practitioner or Physician Assistant license in Georgia & current board certification
- 5+ years of progressive care experience
- BLS and ACLS certified
- Valid driver's license, car insurance, and access to a car
- Strong computer skills, including Word, Excel, and PowerPoint
Responsibilities
- Deliver primary care to patients in their homes
- Engage patients into the WellBe program
- Perform initial patient screenings and physical examinations
- Address urgent care needs of patients
- Collaborate with the community team to complete care assessments
- Order, interpret, and analyze diagnostic tests
- Prescribe and manage medications and treatments
- Update patient records and ensure accuracy
- Deliver palliative care and end-of-life care when needed
- Facilitate family meetings to assist with complicated family dynamics and goals of care
