Job Details
- Employment Type: Permanent
- Hours Per Week: 40
- Schedule: Full-time, Monday to Friday, 8 AM - 5 PM
- Patient Visits: 2 to 6 patients per day in their homes
- Compensation: $110,000 - $125,000 per year, depending on experience
Job Requirements
- Minimum Master’s Degree in Nursing
- Valid Nurse Practitioner or Physician Assistant license in Georgia & current board certification
- 5+ years of progressive care experience required
- BLS and ACLS certified and an NPI# as an NP
- Valid driver’s license, car insurance, and access to a car
- Willingness to drive to patient homes/patient locations
Responsibilities
- Deliver primary care to patients in their homes
- Conduct initial patient screenings and physical examinations
- Address urgent care needs of patients as required
- Collaborate with the community team to complete care assessments
- Order, interpret, and analyze diagnostic tests
- Prescribe and manage medications and treatments
- Update patient records and ensure accuracy
- Provide educational resources to staff, patients, and families
- Deliver palliative care and end-of-life care when needed
- Facilitate family meetings with the WellBe community team
