Nurse Practitioner (NP) - Home Visit

We are assisting a non-profit, independent, community-based organization in the Burlington County, NJ area, seeking experienced Nurse Practitioners to join their team.

This organization is committed to delivering exceptional care across various settings, including home visits, hospitals, long-term care (LTC) facilities, and assisted living (AL) environments. They prioritize both clinical excellence and outstanding patient experiences, supported by a provider-led model and a strong, value-driven ethos.

Day in the Life:

• Conduct majority home visits and some facility visits, manage patient care, and address symptoms.
• Provide referrals, orders, and coordinate with specialists.
• Provide a health assessment appropriate to the visit being performed, as well as diagnose and treat patients who require management of symptoms

Schedule:

• Full-time, Monday to Friday, 8:30 AM - 4:30 PM (with some flexibility)

Compensation/Benefits:

• $110,000 - $135,000/year base salary
• Quarterly bonuses up to $3,125

• Mileage reimbursement ($0.67 per mile)

Qualifications:

• Current, unrestricted NP license
• Palliative Care, Hospice, Geriatrics, or Long-Term Care experience preferred
• Ability to obtain DEA licensure
• Minimum 1 year of NP experience
