Nurse Practitioner – Primary / Palliative Care – Home Health / Assisted Living Facilities

Our client is seeking a Nurse Practitioner to provide care in patients’ homes as well as assisted living facilities around Bismarck, ND. You are able to use a company vehicle, if preferred.

Schedule:
  • Full time: 40 hours per week, 5 8’s or 4 10’s
  • Part time schedule available as well
  • On-call required one week per month, but this can be done telephonically from home and will reduce to once every two months as they continue to grow
Compensation: $100,000 - $150,000/yr base salary, plus bonus

Nurse Practitioner Responsibilities:
  • Perform comprehensive physical evaluations for a Geriatric population
  • Develop individualized care plans and ensure appropriate follow up through the care continuum
  • Work collaboratively with an interdisciplinary team consisting of Physicians, RN’s, Dieticians, Pharmacists, Psychiatrists, Social Workers, Cardiologists and Podiatrists
  • Prescribe all necessary medications/treatments and educate patients/families about medication usage
Qualifications:
  • Master’s Degree in Nursing
  • Licensed Family Nurse Practitioner or Adult Geriatric Nurse Practitioner in the state of North Dakota
  • 2+ years of experience as a Nurse Practitioner within Home Health, Skilled Nursing or Primary Care
