As a Field Nurse Practitioner (NP) or Field Physician Assistant (PA), you will be responsible for improving patients’ quality of life from the comfort of their homes. The NP or PA will work together with the interdisciplinary team to improve patients’ medical, social, and emotional health.

Responsibilities Include:

• Visiting anywhere from 6-8 patients per day providing primary, urgent, and palliative care to the adult geriatric patient population
• Collaborate with the interdisciplinary team managing the health and wellbeing of patients living in their homes
• Provide primary care to patients in the home and urgent care as needed
• Deliver palliative care, and when needed, end of life care to patients and families
• Perform initial patient screenings/physical examinations and create appropriate assessment/care plans
• Order, interpret, and analyze diagnostic tests and determine the need for follow-up appointments and further treatment options
• Prescribe and manage medications and treatments as appropriate, including but not limited to psychopharmacological agents

Schedule: full time

• Monday – Friday: 8a-5p

Compensation: depending on years of experience

• Base salary $110,000 - $125,000/year
• 10% bonus paid quarterly
• $20k SIGN ON BONUS

Qualifications:

• Unrestricted, active NP or PA license in the state of New York
• Board certified
• Minimum of 1 year of NP or PA-C experience
• Valid drivers license
