Nurse Practitioner - Homecare

Join our dedicated healthcare team as a Nurse Practitioner specializing in homecare. This permanent position offers the opportunity to provide compassionate care to elderly patients with chronic and acute conditions in the comfort of their homes. You will be part of a collaborative team that includes physicians, social workers, paramedics, and care coordinators, all working together to deliver exceptional care.

Job Details

  • Employment Type: Permanent
  • Schedule: Full-time, Monday to Friday, 8 AM - 5 PM
  • Patient Visits: 2 to 6 patients per day
  • Compensation: $110,000 - $125,000 per year, depending on experience
  • Sign-On Bonus: $20,000

Job Requirements

  • Minimum Master’s Degree in Nursing
  • Valid Nurse Practitioner or Physician Assistant license and current board certification
  • 5+ years of progressive care experience
  • BLS and ACLS certified
  • Valid driver’s license, car insurance, and access to a car
  • Strong computer skills, including Word, Excel, and PowerPoint

Responsibilities

  • Provide primary, urgent, and acute care to patients in their homes
  • Maintain accurate and current patient records
  • Schedule and conduct initial, urgent, and follow-up appointments
  • Collaborate with the care team to manage patient health and wellbeing
  • Perform initial patient screenings and physical examinations
  • Prescribe and manage medications and treatments
  • Deliver palliative and end-of-life care as needed
  • Facilitate family meetings and provide educational resources
