We are currently hiring a full-time, permanent Nurse Practitioner in the Girard, PA market.
Advanced Practice Providers Responsibilities Include:
As a Field APP, you will be responsible for improving patients’ quality of life from the comfort of their homes. The APP will work together with the interdisciplinary team to improve patients’ medical, social, and emotional health.

Advanced Practice Providers Responsibilities Include:
As a Field APP, you will be responsible for improving patients’ quality of life from the comfort of their homes. The APP will work together with the interdisciplinary team to improve patients’ medical, social, and emotional health.
- Collaborate with the interdisciplinary team managing the health and wellbeing of patients living in their homes
- Provide primary care to patients in the home and urgent care as needed
- Deliver palliative care, and when needed, end of life care to patients and families
- Perform initial patient screenings/physical examinations and create appropriate assessment/care plans
- Order, interpret, and analyze diagnostic tests and determine the need for follow-up appointments and further treatment options
- Prescribe and manage medications and treatments as appropriate, including but not limited to psychopharmacological agents
- No weekends
