NP - Community Based

We're actively seeking Community-Based Clinicians dedicated to delivering high-quality, personalized care to homebound and vulnerable patients. This role is central to providing both primary and minor urgent care in the home, helping bridge crucial care gaps after hospital discharges.

Position Overview

As a Community-Based Clinician, you’ll work with a small panel of patients, addressing acute care needs, supporting care transitions, and breaking down barriers to effective healthcare. You’ll collaborate with an interdisciplinary team of physicians, social workers, and RNs to bring community-centered care directly to patients’ homes.

Key Responsibilities

• Manage a small panel of homebound patients with a focus on acute care and transitional support.
• Collaborate with an integrated care team to ensure comprehensive, patient-centered care.
• Provide compassionate, in-home care to patients from underserved backgrounds.
