We are partnering with a nationally ranked organization that is seeking a dedicated Nurse Practitioner to deliver value-based care to Medicare adults. In this role, you’ll forge meaningful relationships with patients, prioritizing their long-term health and well-being while helping them avoid hospitalizations.
What You’ll Do:
  • Provide high-quality primary care through routine and acute office visits.
  • Conduct annual wellness visits and health risk assessments.
  • Coordinate care with specialists and lead population health initiatives.
  • Educate patients about their health conditions and treatment plans.
  • Collaborate with a supportive care team in an innovative environment.
What We’re Looking For:
  • Master’s degree in Nursing or Nurse Practitioner certification.
  • National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner.
  • Active state Nurse Practitioner license and DEA license.
  • Preferred experience in primary care.
