Job Description
What You’ll Do:
- Provide high-quality primary care through routine and acute office visits.
- Conduct annual wellness visits and health risk assessments.
- Coordinate care with specialists and lead population health initiatives.
- Educate patients about their health conditions and treatment plans.
- Collaborate with a supportive care team in an innovative environment.
- Master’s degree in Nursing or Nurse Practitioner certification.
- Spanish speaking
- National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner.
- Active state Nurse Practitioner license and DEA license.
- Preferred experience in primary care.
