Join our Akron healthcare team as a dedicated Nurse Practitioner, providing exceptional, value-based primary care to Medicare patients. This permanent position in our clinic offers a collaborative environment focused on quality care and building meaningful patient relationships.
Job Details
- Employment Type: Permanent
- Schedule: Monday - Friday, 8 AM to 5 PM
- Location: On-Site in Akron, OH
Job Requirements
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
- National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC
- Active, non-probationary state Nurse Practitioner license
- Active DEA license
- US Work Authorization
- Minimum of 1 year of experience in primary care: internal medicine, geriatrics, or family medicine preferred
Responsibilities
- Provide exceptional primary care to Medicare patients
- Conduct office visits for routine and acute issues
- Administer annual wellness visits and health risk assessments
- Coordinate care with other healthcare providers, specialists, and agencies
- Lead population health initiatives in coordination with the Care Team
- Assist with phone triage and outreach
- Educate patients on health conditions, care plans, and treatments
- Participate in promotional activities and conduct home visits as needed
