NP - Adult Geriatric - Somerset PA

JOB OVERVIEW:

The Nurse Practitioner will provide high-quality care for elderly patients in rehabilitation and nursing facilities. Responsibilities include direct patient care, preventative services, and managing both acute and chronic health conditions. You will assess symptoms, conduct thorough examinations, and develop personalized treatment plans. Collaboration with physicians may be required for more complex cases.

KEY RESPONSIBILITIES:

• Conducts comprehensive exams for elderly patients, managing both acute conditions and chronic illnesses.
• Orders and evaluates diagnostic tests to address age-specific health needs.
• Prescribes appropriate medications and non-medication treatments based on patient assessments.
• Implements interventions to help patients regain or maintain their health.
• Regularly reviews and tracks the effectiveness of treatments.
• Clearly communicates care plans with patients, families, and the healthcare team.
• Provides guidance to patients and families on treatment plans and healthcare decisions.
• Facilitates smooth transitions between different levels of care or healthcare settings.
• Assists in decision-making related to acute and chronic health conditions for patients and families.
• Collaborates with other healthcare professionals to ensure coordinated patient care.
• Educates patients on managing their conditions independently.
• Performs other related tasks as needed.

QUALIFICATIONS & EDUCATION:

• Graduate of an accredited Nursing School and Nurse Practitioner Program.
• Certified as a Nurse Practitioner (AANP or ANCC).
• Current BLS certification and DEA license.
• Proficient with Electronic Health Records (EHR).
• 1-2 years of Nurse Practitioner experience required.
