Job Title: Family Nurse Practitioner (FNP) / Physician Assistant (PA-C) - Advanced Practice Provider (In-Home Urgent Care) - Value Based Care

Location: Lynnwood, WA 
Sign-On Bonus: $15,000

We’re looking for an experienced and compassionate Family Nurse Practitioner (FNP) or Physician Assistant (PA-C) with at least one year of experience in urgent care, ER, ICU, or primary care settings. What You'll Do: As an Advanced Practice Provider, you’ll deliver high-quality care to patients with chronic conditions using a cutting-edge care model. You’ll perform treatments such as suturing, IV fluid administration, splinting, and wound care—all in a home environment. A Medical Technician will accompany you on patient visits, driving a fully equipped company vehicle so you can focus on patient care.

Key Responsibilities:

  • Perform comprehensive health assessments and diagnose complex illnesses.
  • Provide advanced therapeutic interventions, including minor procedures.
  • Work closely with a Medical Technician who will assist in patient care and logistics.
  • Maintain continuity of care through ongoing patient education and follow-up.
  • Represent the organization professionally and uphold high standards of patient care.

Schedule:

  • Flexible 10-hour shifts, with a minimum of 14 shifts per month (average 140-150 hours).
  • Opportunity to block schedules (e.g., 6 days on, 6 days off).
  • See 6-8 patients per day on average, with start times at 8 AM, 9 AM, 10 AM, or 12 PM.
  • Centralized scheduling support ensures your shifts align with market needs.

Qualifications:

  • Minimum of two years of experience as an APP in the Emergency Department, Urgent Care, Internal Medicine, Hospital, Family Practice, Cardiac, Pulmonary or other relevant areas
  • Those with less than two years of experience as an APP, but with two or more years’ experience as a paramedic, RN (ED or ICU), Flight Nurse or a graduate from an ENP program or fellowship (NP’s) or EM/Hospitalist residency (PA’s) are also encouraged to apply
  • Licensed FNP or PA-C in the state of Washington, with current BLS certification (ACLS preferred).

Our Advanced Practice Providers team with DHMTs (Medical Technicians) and are equipped with everything needed to treat common to complex injuries and illnesses through comprehensive health assessments, including diagnosis, treatment, and outcome management, for patients of all ages. Our innovative model allows us to provide definitive care in the home, including point of care laboratory studies, minor procedures, splinting, wound care, suturing, IV fluid and medication administration. We provide whole person care by facilitating timely follow up and care coordination.
