The Manager of Care Management will organize, coach staff, monitor performance and progress, enforce program standards and ensure quality service for our members
Responsibilities:
- Select and develop a team of Care Managers and provide ongoing coaching and mentorship
- Set performance goals and evaluate team progress in line with organizational mission and goals
- Address performance, quality and other opportunities as they arise
- Create policy and procedure to improve care for our patients
- Work seamlessly with the multi-disciplinary team as well as the organization's leadership team
- Intervene as needed to avoid unnecessary hospital visits
- Develop and maintain strong relationships for our member's needs
Qualifications:
- 4+ years in a leadership role as a Registered Nurse guiding Care or Case Managers
- 2+ years of experience in an ambulatory or outpatient setting
- Experience working with patients who have complex medical and behavioral conditions
- Bachelor's degree required; Master's degree preferred
- Case Management Certification required
- Knowledge of compliance standards and NCQA
- Excellent communication skills both verbal and written
