Manager of Care Management – Population Health

GHR Search is currently representing a client in the Greater Baltimore Metro Area that provides the community with patient-centered care for primary care, chronic conditions and behavioral health needs. Their Care Model includes an interdisciplinary approach to each individual served using evidence based practice combined with population health data. They are current seeking a highly compassionate and professional Nursing Leader to join their team as the Manager of Care Management in their Baltimore County Clinic.

The Manager of Care Management will be a licensed Registered Nurse or a Licensed Social Worker with at least 5 years of Care Management experience working within the community health, skilled nursing, acute care or primary care.

Areas of Accountability include:

  • Management of the Care Management Program and the staff members of care management team
  • Organizes work groups, monitors performance and progress and coaches staff members on quality patient care delivery and service excellence
  • Recruit, hire, train and supervisor all members of the Case Management Team
  • Collaborates with interdisciplinary care team and participates in care rounds with providers, nursing staff and outreach team
  • Creates and maintains positive relationships with area SNF’s, hospitals and community care organizations regarding public and community health topics as well as developing strategic partnerships to meet the needs of the members
  • Responsible for all clinical, care management and daily operations related to the care management team
  • Will oversee a team of 10-12 direct reports including Care Manager RNs, Social Workers, Homebased Care Manager and Transitional Care Managers.
  • Reports to the National Director of Case Management


  • Monday to Friday Business Hours Schedule with some flexibility required

  • Valid Registered Nurse license in the state of Maryland or Valid Social Worker License
  • 5 years of experience working in complex case management within an acute care, community care or population health setting
  • At least 3 years of leadership experience working in Public health, SNF or acute care
  • Ability to adapt and respond to an ever changing, fast paced and results-oriented environment
  • Knowledge of NCQA programs and experience working with compliance, standards and re-accreditations
