Job Details
- Employment Type: Permanent
- Salary: $124,800 annually
- Hourly Rate: $60–65/hour
- Shift: 10-hour days, 12 p.m. – 10 p.m., with rotating weekends (every 6-8 weeks)
- Location: On-Site in New York
Job Requirements
- Associate of Science in Nursing (ASN) required; Bachelor of Science in Nursing (BSN) preferred
- Current, active Registered Nurse (RN) licensure in good standing and willingness to be licensed in multiple states
- 3-5 years of acute nursing experience and 2-3 years of RCC experience preferred
- Reliable transportation and proof of liability insurance
- Ability to support after-hours schedules and on-call needs
- Care Coordination and Transition Management (C.C.C.T.M) certification preferred
Responsibilities
- Complete patient admissions and serve as a resource for other Recovery Care Coordinators
- Lead communication and coordination of care between all members of the healthcare team
- Act as a patient advocate and assist in the development of personalized care plans
- Support Market Manager in staffing coordination and ensure resources are available
- Provide guidance and support during times of urgency or critical needs
- Assist in recruiting, interviewing, and training new team members
- Maintain data and reporting accuracy
- Identify and solve for negative trends and hurdles in the healthcare system
