Director of Nursing – Skilled Nursing Facility

We are seeking a Director of Nursing to oversee and support clinical excellence in a 30-bed skilled nursing facility. This role offers the opportunity to travel across multiple communities while ensuring regulatory compliance and best-in-class care.
The Director of Nursing (DON) is responsible for the overall leadership and clinical management of skilled nursing services. This includes accurate completion and oversight of the Resident Assessment Instrument (RAI) process, managing individualized care plans, and ensuring adherence to all federal, state, and local regulations.

Key Responsibilities:
- Lead and manage the clinical operations of skilled nursing across assigned communities
- Oversee completion of resident assessments (MDS) and individualized care plans in compliance with all applicable regulations
- Support implementation of the Resident-Centered Care model
- Serve as a resource and mentor to clinical team members, including training, coaching, and performance management
- Ensure accurate documentation of services and timely updates to care plans
- Collaborate with interdisciplinary teams to identify and meet residents' ongoing care needs
- Participate in recruiting, hiring, and onboarding of nursing staff
- Promote quality assurance and continuous improvement initiatives
- Travel regularly across multiple states and sites to support operational excellence
- Graduate of an accredited school of nursing
- Active RN license in good standing (multi-state licensure or eligibility for reciprocity preferred; costs for reciprocity may be covered)
- Minimum two (2) years of RN experience, with at least one (1) year in long-term care, home health, or assisted living
- At least one (1) year of nursing supervisory experience, including staff management and department operations
- Knowledge and experience in MDS and Care Plan development strongly preferred
- AANAC (American Association of Nurse Assessment Coordinators) certification preferred or willingness to obtain within 90 days of hire
- Strong interpersonal, decision-making, and problem-solving skills
- Proficiency with EMR systems and Microsoft Office Suite
- Ability to travel weekly, with flexible hours including weekends or evenings as needed
