Complex Case Manager RN

A large, reputable healthcare system in the New Orleans Area is seeking to hire several Field-Based Registered Nurse Case Managers and Care Coordinators to join their Team as the program has significantly increased in patient census.

This role is NOT home health nursing/no direct patient

care (i.e. wound care, IVs, etc.) & no Oasis documentation required

Responsibilities:

• The Field-Based Care Manager will be responsible for complex case management & care coordination post discharge from an inpatient facility
• You will be coaching, educating and supporting vulnerable or at risk members post discharge
• You’ll work with PCPs in the community and our organization’s providers in sharing the care plan to create further engagement in the care for our members

Schedule: Monday through Friday, business hours

Salary Range: $80,000 to $89,000 annually

Qualifications:

• 3+ years of case management experience
• Licensed Registered Nurse
• CCM highly preferred
• InterQual and/or Milliman Care Guidelines experience preferred
• Experience with Medicaid and Medicare managed care preferred
• Knowledge of local population, geography, and resources is preferred
• Experience working with complex cases
