Nurse Practitioner - Community-Based

Join our Baltimore healthcare team as a dedicated and compassionate Community-Based Nurse Practitioner. This permanent role involves providing primary and urgent care to homebound members, focusing on managing transitions of care, especially post-hospital discharge. You will practice enhanced primary care and benefit from expert care management, significantly impacting the lives of vulnerable patients.

• Location: Baltimore, MD
• Employment Type: Permanent
• Hours Per Week: 40
• Compensation: $125,000 - $150,000/yr (Depending on Experience)

Job Requirements

• Certified Registered Advanced Practice Provider in Adult Medicine with Acute Care experience
• ACLS certified
• Willingness to conduct home visits
• Experience with Electronic Medical Records and Microsoft Office
• Patient-centered focus with a creative approach to problem-solving
• Familiarity with care transitions and chronic condition management
• Preferred: Experience in a Patient-Centered Medical Home and working with high-risk, medically complex patients

Responsibilities

• Provide community-based medical and care-coordination services for recent hospital discharges
• Manage a small panel of homebound members requiring primary care
• Resolve acute issues in the home and clinic or direct members to effective solutions
• Ensure timely and accurate completion of chart documents
• Collaborate with the Core Clinical Team and Integrated Care Team for problem-solving and care coordination
• Identify and refer to specialty programs for issues such as abuse/neglect and food insecurity
• Implement transitional care protocols and help patients navigate barriers to health and life goals
• Educate and coach for medication adherence and chronic disease self-management
• Monitor members at risk of being lost to care and implement interventions to prevent loss
