Job Description

We are seeking a highly skilled and compassionate Nurse Practitioner to conduct in-home wellness risk adjustment assessments for Medicare members and other at-risk populations. Job Details
- Employment Type: Permanent, Full-Time
- Salary: $120,000 - $150,000 annually
- Hours Per Week: 40
- Shift Duration: 8 Hour Days
- Specialty: Homecare
Job Requirements
- Must have a valid, unencumbered Nurse Practitioner License for the state of practice
- Previous in-home risk assessment experience preferred
- 3 years of patient care experience preferred (primary care/adult/geriatric, EMR)
- Access to reliable transportation for travel within designated areas
- Strong ability to work within an EMR system
- Ability to work independently
- Bilingual is a plus
Responsibilities
- Perform annual in-home wellness visits and risk adjustment health assessments
- Provide individualized quality care to the elderly population in their homes
- Deliver patient health education on topics such as pain management and medication
- Build relationships of trust with members through exceptional communication and empathy
- Assist in closing quality care gaps (i.e., screenings and labs)
- Collaborate to enhance care delivery
- Other duties as assigned
