Travel Nurse Practitioner - Advanced Practice Provider (APP)

Join our dynamic healthcare team as a Travel Nurse Practitioner, specializing as an Advanced Practice Provider (APP) in Homecare. We are seeking a compassionate and skilled Nurse Practitioner to conduct in-home wellness risk adjustment assessments for Medicare members and other at-risk populations. This travel position offers the flexibility of a 1099 (PRN) role, allowing you to balance competing priorities while making a significant impact on patient care.

  • Travel Requirements: More than 50%
  • Pay Rate: $110 per completed in-home assessment

Job Requirements

  • Must have a valid unencumbered NP License for the state you will be working in
  • This role requires travel within the assigned job posting county up to a maximum 55-mile radius
  • Previous in-home risk assessment experience preferred
  • Previous 1099 (PRN) experience is preferred but not required
  • 3 years of patient care experience preferred (primary care/adult/geriatric, EMR)
  • Access to reliable transportation that will enable you to travel to member's homes within a designated area
  • Strong ability to work within our EMR system
  • Ability to work independently
  • Bilingual is a plus

Responsibilities

  • Perform annual in-home wellness visits and risk adjustment health assessments on a population with chronic conditions
  • Provide individualized quality care to the elderly population in their homes
  • Deliver patient health education on topics such as pain management and medication
  • Build relationships of trust with members through exceptional communication and empathy
  • Assist in closing quality care gaps (i.e., screenings and labs)
  • Collaborate to enhance care delivery
  • Commit a minimum of 30 hours per month
