Join Our Team as an Urgent Care Provider – Make a Real Impact in Patient Care!

Are you a skilled medical professional passionate about immediate care and chronic condition management? Join our dynamic team as an Urgent Care Provider and make a meaningful impact by offering timely, compassionate care for acute illnesses and chronic condition flare-ups.

 

Locations: Clevland & Columbus OH

(paid per visit) Pay rate: $85.00 Hourly.

Shift: M-F 8-5 are the typical hours and would consider weekends only

 

Key Responsibilities:

  • Clinical Care: Handle a variety of acute medical issues, including:
    • New illness onset
    • Exacerbations of chronic conditions (e.g., CHF, Asthma/COPD)
    • Shortness of breath, hyperglycemia, elevated blood pressure
    • Infections, bronchitis/pneumonia, wounds, aches, pains
    • Mental health concerns (depression/anxiety)
    • Immediate care to avoid ER visits
    • Hospital follow-ups when Primary Care isn't available
  • Care Coordination:
    • Complete and accurate chart documentation
    • Collaborate with Treatment and Triage Nurse for sick calls and visits
    • Define visit goals with the Core Clinical Team
    • Communicate post-visit follow-up to Core Clinical Team
    • Work with Integrated Care Team (behavioral health, nutrition, health education) for comprehensive care
    • Refer to specialty programs in cases of instability or crisis
    • Implement transitional care protocols
    • Help patients navigate barriers to health and life goals
    • Adjust care plans based on evidence-based chronic disease management
    • Educate and coach on medication adherence and self-management skills
    • Monitor and intervene for patients at risk of being lost to care

What We’re Looking For:

  • Certified Registered Advance Practice Provider in Adult Medicine with Acute Care experience
  • ACLS certified.
  • Creative problem-solving skills to help patients overcome care barriers
  • Ability to manage multiple priorities and deadlines
  • Adaptability to a fast-paced, results-oriented environment
  • Proficiency with Electronic Medical Records and Microsoft Office
  • Patient-centered focus with a commitment to care transitions and reducing readmissions

Preferred Qualifications:

  • Experience in a Patient Centered Medical Home
  • Expertise in managing high-risk and medically complex patients with multiple comorbidities
