A highly reputable, multi-site healthcare organization is seeking an experienced Home Health Clinical Manager to join their team! This is a great opportunity for a seasoned home health RN and Leader that is looking to take the next step in their career, while working within a unique, patient-centered program that provides care for patients on-site, throughout the community and in their home environment.
This is a very unique program, specialized in providing all-encompassing health care services for the adult and geriatric population in an effort to keep them healthy, involved in the community and prevent them from being admitted or readmitted into hospitals or skilled nursing facilities.
While reporting to the Director of Clinical Services, the Home Health Clinical Manager will be responsible for clinical and operational oversight of the professional nursing staff, home health aides, nursing assistants. Additional responsibilities include:
Schedule: Full-time, Monday – Friday, Days
Compensation: $85,000 - $95,000/yr

While reporting to the Director of Clinical Services, the Home Health Clinical Manager will be responsible for clinical and operational oversight of the professional nursing staff, home health aides, nursing assistants. Additional responsibilities include:
- Manages daily clinical operations while interfacing and collaborating with staff, leadership, and the Interdisciplinary Team (IDT).
- Evaluates, develops, mentors, coaches, counsels, and disciplines department staff.
- Identifies staff education needs. Provides education and oversight regularly based on departmental needs.
- Actively participates as a member of the interdisciplinary team (IDT) in ongoing participant assessment and monitoring.
- Attends care planning meetings to report personal care services, patient self-care abilities, and changes noted in participant condition.
- Oversees the coordination of staffing to meet day center needs in partnership with Clinical Center Manager.
- Compiles and maintains inventory and par levels for individual Participant (Ppt) supplies.
- Utilizes resources to meet department budget through oversight for department hours and schedules, payroll, mileage accuracy, and management of overtime.
- Oversees the care provided to ensure patient safety and adherence to the care plan.
- Participates in Quality Program, Elder Council, and other leadership meetings.
Schedule: Full-time, Monday – Friday, Days
Compensation: $85,000 - $95,000/yr

Qualifications:
- NJ Licensed Registered Nurse
- A minimum of 5 years’ experience as a Registered Nurse, preferably with the adult and geriatric population in community based, outpatient, home health or long-term care settings
- BSN required. ASN plus 7 years of experience in lieu of BSN.
- At least 2 years of Supervisory nursing experience
- At least 1-2 years of home health / home care / visiting nurse experience.
