Personal Care/Memory Care Manager (RN)
GHR Search is currently partnered with a well-known and highly reputable CCRC in Delaware County, PA. This 4-star, family-oriented facility is seeking a Wellness Manager for its Personal Care/Memory Care divisions.
As the Wellness Manager, you will oversee the care performance of the teams within Assisted Living and Memory Care. This is a wonderful opportunity to strengthen your leadership skills within a large, growing network of senior living and health care campuses.
Key Duties:
- Reports to the Director of Nursing (DON)
- Responsible for compliance with state, local, and company policies, procedures, and regulations
- Coordinate and provide clinical oversight and care to residents including: medication management, pain management, wound care, nutrition, and respiratory care
- Ensure resident’s individualized service plan is maintained/updated to reflect current needs in collaboration with other team members
- Organize and monitor outside support services such as hospice, therapy, and home health
- Actively participates in continuing care meetings
Schedule: Full-Time, Regular
Salary & Compensation:
- $80,000 - $110,000, depending on experience

Candidate Qualifications:
- Pennsylvania licensed Registered Nurse
- At least 5+ years of experience as an RN
- At least 1+ year of Management experience within a Home Health, Assisted Living, or Memory Care setting
- Experience and comfort working with an EMR system
