Description: We provide concierge health services using a PCP-driven care model for the most clinically complex and vulnerable community members. Our holistic approach addresses both medical and psychosocial issues to improve outcomes for high utilizers of healthcare services.
Pay Rate: $70-$80 Hourly (paid weekly and eligible for health benefits)
Shift: Monday – Friday (8a – 5p with 1hour lunch)

Job Summary: The same day care provider manages acute illnesses, exacerbations of chronic conditions, and reassures members that ER visits are not always necessary. This role supports long-term chronic condition management and collaborates with the PCP and Integrated Care Team to reduce ER utilization, decrease hospital admissions, and enhance member satisfaction.
Duties and Responsibilities: Clinical Care:
- Manage acute illnesses and chronic condition exacerbations.
- Address shortness of breath, CHF, asthma/COPD, infections, hyperglycemia, and elevated blood pressure.
- Conduct hospital follow-ups and urgent evaluations.
Coordination of Care:
- Complete accurate chart documentation.
- Collaborate with Treatment and Triage Nurses.
- Communicate with the Core Clinical Team and Integrated Care Team.
- Refer to specialty programs as needed.
- Implement transitional care protocols.
- Educate on medication adherence and chronic disease management.
- Prevent loss to care for at-risk members.
Education & Experience Requirements:
- Certified Registered Nurse Practitioner or Physician Assistant in Adult Medicine with Acute Care experience.
- ACLS certified.
- Creative problem-solving skills and ability to manage multiple priorities.
- Proficient in Electronic Medical Records and Microsoft Office.
- Patient-centered focus with experience in care transitions and chronic condition management.
Preferred Qualifications:
- Experience in a Patient Centered Medical Home.
- Experience with high-risk, medically complex patients with multiple comorbidities.
